Consent to Personal Data Processing

Hereby, I give my consent to O'O'BQ "OTABEK HASANOV", registered in Uchkuprik District, Fergana Region (hereinafter referred to as the “Intermediary”), and to other organizations authorized by the “Intermediary”, within the territory of the Republic of Uzbekistan as well as outside its territory, with or without the use of automation tools, to process my personal data, namely collecting, recording, systemizing, accumulating, storing, clarifying (updating, changing, supplementing), extracting, using, transferring to third parties, including transferring and distributing to the territory of foreign states that provide and do not provide adequate protection for the rights of personal data subjects, posting on public Internet resources – on the website pages of the “Intermediary”, depersonalizing and destroying personal data, including, but not limited to:

first name, last name, and patronymic;

biometric data;

date of birth;

residential address;

passport details;

contact phone number;

email address;

bank card details, as well as any monetary transactions carried out using bank cards

data stored on the mobile device and used within the scope of providing specific functions, services, and features through the mobile application, including information transmitted via cookies and data related to granting access rights.

For the following purposes:

ensuring compliance with the legislation of the Republic of Uzbekistan and internal normative documents;

concluding and executing agreements to which I am a party, including taking measures to monitor and forecast current and future fulfillment of obligations under agreements;

requesting any information about me at any time from any source in any amount in accordance with the legislation of the Republic of Uzbekistan;

conducting statistical and/or marketing research by the “Intermediary”, providing information on promotions, surveys, inquiries, and/or new products/services;

contacting me, including sending notifications, requests, and information regarding the use of products/services, as well as processing requests and applications received from me, including subsequent submission of such requests and applications to counterparties for execution;

preventing actions that violate the requirements of legislation or agreements.

This consent is given for an indefinite period.

I am aware of my rights provided for in Article 22 of the Law of the Republic of Uzbekistan dated 02.07.2019 No. ZRU-547 "On Personal Data", namely my right to obtain information regarding the processing of personal data, which includes the following:

confirmation of the fact of personal data processing;

legal grounds and purposes of personal data processing;

methods used in personal data processing;

the name and location of the operator, information about persons (excluding operator employees) who have access to personal data or to whom personal data may be disclosed based on a contract with the operator or by law;

the composition of processed personal data related to the relevant personal data subject, the source of their obtaining, unless another procedure for providing such data is provided for by law;

terms of personal data processing, including their storage periods;

the procedure for the personal data subject to exercise rights provided for by the Law of the Republic of Uzbekistan "On Personal Data";

information on completed or intended cross-border data transfers. I know that I can revoke the consent given for the processing of personal data by submitting an application to the “Intermediary” in any form.
